He was born June 15, 2005, in Bloomington, a son of Nicholas J. and Kimberly J. Modine Phillips.

Surviving are his parents, Nick and Kim Phillips, DeWitt; a brother, Jacob Phillips, DeWitt; his maternal grandparents, Jan and Jim Modine, Hudson; his paternal grandmother, Linda Kampfe, Las Vegas; an aunt and uncle, Kelly and Charlie Payton, Hudson, and Jason's cousin, C.J.

He was preceded in death by his paternal grandfather, John A. Phillips of Moline.

Jason was a member of Trinity Lutheran Church and attended Trinity Lutheran School. An avid learner, he never missed a day of school up until his diagnosis in November 2014. Jason was an adventurous outdoorsman with a love of hiking, four-wheeling, fishing, swimming and helping his dad with various projects. The consummate foodie, he was always willing to try new and different foods. Jason was a great conversationalist and a bit of an old soul who took great pleasure in meaningful conversation.

A lifelong animal lover, his ultimate dream was to be a veterinarian and rescue every animal he could.

He will be remembered for his great sense of humor … he was always entertaining and looking to put a smile on someone’s face. More than anything, Jason was a young man of strong faith who had a huge heart and a deep, abiding love for his family.
